膀胱根治性切除术前新辅助化疗:为什么必须坚持?

摘要

目的：本研究对肌肉浸润性膀胱癌(MIBC)新辅助治疗领域的最新技术和新策略进行了重要的文献综述。 方法:采用PubMed、Scopus和Clinical Trials.gov进行非系统文献回顾，检索过去15年与MIBC新辅助治疗相关的文献。包括前瞻性和回顾性研究。 结果：以铂为基础的治疗是金标准，主要包括顺铂与长春新碱、甲氨蝶呤、阿霉素、吉西他滨、阿霉素甚至表柔比星的组合。MVAC的5年绝对总生存率提高5%，绝对无病生存率提高9%。CMV治疗可使10年总生存率从30%提高到36%，死亡率降低16%。吉西他滨+顺铂方案提供20%的病例完全缓解，与MVAC方案相比，肿瘤预后不差。最近研究新辅助免疫治疗的前瞻性试验显示，阿特朱单抗的完全缓解率为29%，派姆单抗为39.5%。酪氨酸激酶抑制剂途径正在探索中，可以提供一个有趣的策略来改善生存结果。 结论：现有证据表明，在根治性膀胱切除术前接受新辅助治疗的MIBC患者有更好的肿瘤预后。虽然MVAC仍然是符合顺铂条件的患者的标准治疗，但针对不符合顺铂条件的患者的新策略正在开发中，因此免疫治疗似乎有很大的希望。
