前庭刺激对痴呆平衡障碍的临床应用

摘要

背景:在之前的一项关于阿尔茨海默病(AD)的研究中,我们发现由大脑疾病引起的前庭功能障碍与平衡障碍有关。以前没有临床研究试图通过使用空气热量装置的前庭刺激来防止痴呆患者平衡障碍的进展。 目的:本初步研究的目的是通过诱导前庭补偿,特别是通过前庭刺激激活大脑,延缓平衡障碍的进展。 方法:15名受试者被随机分为刺激组和非刺激组。刺激组8例接受前庭刺激,每2周进行一次,为期6个月。7例AD患者参加无刺激组(对照组)。在研究开始前和6个月后,对两组进行了小型精神状态检查(MMSE)、步进测试、热量测试和平滑追踪眼动测试。 结果:在平衡参数方面,两组之间的各种测试没有显示出任何显著性差异。然而,在步进试验中,非刺激组的下降率往往高于刺激组。刺激组的MMSE评分下降率低于非刺激组(p=0.015)。本研究期间未追踪不良事件。 结论:重复的前庭刺激可以帮助患者保持更大的平衡性和更高的功能。为了证明这些效果,未来的临床应用将需要更多的病例和更长的时间的前庭刺激。本研究表明,空气热量装置刺激AD患者前庭神经是安全且可耐受的。
