结肠炎相关结直肠癌(CAC)仍然是溃疡性结肠炎(UC)的一个重要并发症，死亡率约为15%，这使得早期诊断CAC至关重要。目前的监测标准，包括重复的结肠镜检查和活检粘膜样本的组织学检测，是繁重和昂贵的，因此需要更少的侵入性方法和可靠的生物标志物。在这一领域进行了大量的研究，取得了显著的进展，但至今尚未建立临床相关的生物标志物。本文综述了CAC与散发性结直肠癌的遗传和分子差异，以及CAC癌变早期检测的研究进展。它专注于正在开发的生物标记物，这些标记物可以很容易地在体液或呼吸样本中进行测试，一旦临床可用，将有助于区分进展者(UC患者将发展成发育不良)和非进展者，并使早期干预降低癌症发展的风险。
