细胞角蛋白和细胞外胶原蛋白表达在膀胱癌预后中的价值

摘要

背景:胶原蛋白的表达水平与各种癌症的进展以及与细胞角蛋白的相互作用有关,但在膀胱癌(BC)中尚未得到充分的研究。因此,我们分析了1A1胶原蛋白(Col1a1/COL1A1)、3A1胶原蛋白(col3a1/COL3A1)、5A2胶原蛋白(col5a2/COL5A2)、细胞角蛋白14 (krt14/CK14)和细胞角蛋白17 (krt17/CK17)在不同分期BC尿路上皮标本中的基因和蛋白表达水平。 方法:对102份新鲜冷冻标本和190份福尔马林固定石蜡包埋(FFPE)标本进行免疫组化和RT-qPCR检测。表达水平与临床病理及随访数据相关。 结果: Col1a1, col3a1, col5a2 和 krt14 mRNA水平在高级别和肌肉浸润性BC (MIBC)中显著过表达,而在低级别和非肌肉浸润性BC (NMIBC)中显著过表达。col1a1 (p = 0.004)、col3a1 (p = 0.004)和col5a2 (p = 0.028)高表达患者的疾病特异性生存期(DSS)较短。与NMIBC相比,MIBC中CK14 (p = 0.020)、COL3A1 (p = 0.006)和Col5A2 (p = 0.006)蛋白表达水平显著升高,而CK17蛋白表达水平显著降低(p = 0.05)。此外,高级别BC中CK14 (p = 0.002)和COL5A2 (p = 0.006)蛋白表达水平显著高于低级别BC。COL5A2和CK14高表达组DSS较短(p = 0.033), CK14高表达组DSS较短(p = 0.042)。 结论:胶原和细胞角蛋白的表达变化是BC的单变量预后标志。
