背景:关于是否使用抗精神病药会增加血栓栓塞的风险,不同的临床研究结果不一致。在本文中,我们回顾了所有相关文献,为临床诊断和治疗提供了建议。 方法:截至2019年6月,对PubMed,Web of Science,EMBASE,MEDLINE,Cochrane和Scopus数据库进行了彻底搜索。两名研究人员独立搜索文献,提取了数据。数据通过Stata 12.0软件进行分析。 结果:共纳入22个研究,涉及31514226个受试者。这项荟萃分析表明,服用第一代或第二代抗精神病药的患者发生静脉血栓栓塞和肺栓塞的风险高于未使用抗精神病药的患者,低效的第一代药物比高效的抗精神病药增加静脉血栓栓塞的风险,奥氮平,氯氮平,氟哌啶醇,奋乃静和利培酮也显着增加了其风险。肥胖人群静脉血栓形成的风险高于超重人群,不少于65岁的患者比年轻患者发生静脉血栓的风险更高。此外,服用抗精神病药的妇女比男性有更高的肺栓塞风险。 结论:使用抗精神病药会增加静脉血栓栓塞和肺栓塞的风险,这将受到抗精神病药类型和患者特征的影响。
