贫血与痴呆症的关联:台湾全国性的队列研究

背景:除传统的风险预测因素外,贫血是否是痴呆的早期生物标志物,还需要确定。 目的:这项基于人群的队列研究旨在使用台湾国家健康保险研究数据库中的数据调查新诊断为贫血患者的痴呆风险。 方法:所有没有脑卒中住院史,除痴呆,精神病,脑外伤,大手术或失血性疾病之外均无中枢神经疾病史的新诊断贫血患者(n = 26,343)被纳入研究。还包括一组非贫血对照,根据人口统计学和合并症与贫血患者按1:4比例配对。与竞争性对照相比,竞争性风险分析用于评估贫血患者的痴呆风险。 结果:贫血患者痴呆风险调整后的亚分布危险比(SHR)为1.14(95%置信区间[CI]:1.08〜1.21,p <0.001)。与不使用铁补充剂的患者相比,使用铁补充剂的患者倾向于表现出较低的痴呆风险(调整后的SHR:0.84; 95%CI:0.75〜0.94,p = 0.002)。亚组分析显示,痴呆症与贫血之间存在正相关关系,女性,70岁及以上的女性以及无高血压,糖尿病或高脂血症的患者。 结论:当前基于人群的队列研究确定了新诊断的贫血是痴呆的危险因素,而且补充铁能够降低缺铁性贫血患者痴呆的风险。
