背景:慢性肝病(CLD)是由酗酒、非酒精性脂肪肝或病毒介导的肝炎等生活方式引起的,影响着世界上很大一部分人口。慢性肾衰竭进展为终末期疾病,死亡率高。肝移植是这类终末期疾病患者唯一被批准的治疗方法。然而,肝移植的数量是有限的,因为可供选择的合适的供体是有限的,而且进行这项手术的费用极高。在这种情况下,干细胞(SC)介导的肝再生已成为一种潜在的治疗替代方法。 目的:本文旨在批判性地分析基于干细胞的干预治疗终末期肝病的现状和未来展望。对临床研究、作用机制和今后的发展方向进行了探讨。 方法:在https://clinicaltrials.gov.in和http://www.isrctn.com上搜索临床试验数据库,以识别随机、非随机和对照研究进行的关键字如“肝脏疾病和间充质干细胞(MSCs)”,“肝硬化和MSCs”和“肝脏疾病和SCs”。此外,我们还在https://www.ncbi.nlm.nih.gov/pubmed/数据库中搜索了类似的关键字,以查找可用的报告及其关键分析。 结果:搜索结果产生了大量使用骨髓来源干细胞、MSCs和肝细胞的研究。研究表明,SCs通过抗炎症、自身免疫抑制、血管生成和抗凋亡等机制在肝保护过程中发挥关键作用。此外,研究表明SCs来源的旁分泌因子促进血管生成,减少炎症反应和抑制肝细胞凋亡。 结论:以SC为基础的干预可显著改善CLD患者;然而,有必要对长期随访进行随机对照研究。
