肥胖是2型糖尿病的主要危险因素。2型糖尿病的治疗除了血糖参数外,还需要体重管理。对于肥胖的2型糖尿病患者,二甲双胍、葡萄糖共转运蛋白-2抑制剂或胰高血糖素样肽-1受体激动剂应作为控制高血糖和体重或脂肪分布的第一要务。这些药物与磺脲类、噻唑烷二酮类和胰岛素的结合也可能是慢性疾病的需要。这些药物会导致体重增加。幸运的是,与合成药物相比,许多对糖尿病和肥胖症有好处的植物化学物质的副作用最小。本文综述了控制血糖和控制体重的治疗策略,重点介绍了抗糖尿病药物和植物化学物质。此外,还讨论了抗糖尿病药物和植物化物对葡萄糖激动剂的作用、腺苷激活蛋白激酶的激活和脂肪细胞的靶向性。

摘要: Obesity is a prominent risk factor for type 2 diabetes. Management of type 2 diabetes requires weight management in addition to glycemic parameters. For obese type 2 diabetes patients, metformin, Sodium-glucose co-transporter-2 inhibitors or Glucagon-Like Peptide-1 Receptor Agonists should be prescribed as the first priority for controlling both hyperglycemia and body weight or fat distribution. The combination of these drugs with sulfonylureas, thiazolidinediones, and insulin may also be required in chronic cases. These drugs cause weight gain. Fortunately, many phytochemicals having a beneficial effect on diabetes and obesity, have minimum side-effects as compared to synthetic drugs. This review discusses the treatment strategies for controlling glycemia and weight management, with the focus on anti-diabetic drugs and phytochemicals. Glucagonostatic role, activation of Adenosine monophosphate-activated protein kinase and adipocyte targeting potential of anti-diabetic drugs and phytochemicals are also discussed.
