背景:心肌氧化还原状态是心房生物学,调节心肌细胞凋亡,离子通道功能和心脏肥大/纤维化和功能的关键决定因素。然而,目前尚不清楚心房氧化还原状态的靶向是否是预防心房颤动的合理治疗策略。 目的:探讨心房氧化还原状态和抗氧化治疗在心房颤动中的作用。 方法:在Medline发表的文献中寻找将心肌氧化还原状态与心房颤动发病机制联系起来的实验和临床证据,以及研究重新定位治疗在预防心房颤动中的作用的研究。 结果:来自动物模型的数据显示,改变的心肌亚硝基氧化还原平衡和NADPH氧化酶活性与心房颤动的发病机理有因果关系。类似地,实验动物数据支持心房组织中增加的活性氧/氮物种形成与心房肌细胞的电生理学特性改变和电重构相关,有利于心房颤动的发展。在人类中,使用氧化还原相关治疗方法(例如他汀类药物或抗氧化剂)的随机临床研究未证明在预防心房颤动发展(主要是术后心房颤动风险)方面有任何益处。 结论:尽管有强大的实验和转化数据支持心房氧化还原状态在心房颤动发病机制中的作用,但在使用氧化还原相关疗法的随机临床研究中,这种机械证据尚未转化为心房颤动风险的临床益处。
