血栓闭塞性脉管炎的危险因素、机制和治疗:近期研究综述

背景:血栓闭塞性脉管炎(TAO)是一种非动脉粥样硬化性血栓形成性血管炎,累及下肢中小血管。目前尚没有明确的病因病机,需要更有效的治疗。目的:总结和综述TAO的危险因素、发病机制和治疗方法的最新研究进展,并以图表的形式整理相关信息,提供较为完整的参考。 目的:我们在PubMed上搜索了没有文章类型限制的关于TAO的英文文献,包括近10年来关于TAO的危险因素、病理机制和治疗的文章,并提供必要的补充(超过范围的参考文献和俄罗斯文献的英文摘要)。 结果:筛选文献内容后,对99篇参考文献进行评价。我们发现TAO的危险因素包括吸烟、基因因素和牙周病。TAO的作用机制涉及氧化应激、免疫、血流动力学变化和炎症反应等。此外,牙周病与TAO的遗传因素和吸烟相关性存在相似性,二者之间的关系还有待进一步研究。对于TAO的治疗,还提到了药物治疗、血管内介入和血管重建手术、自体细胞治疗和新疗法。此外,还可以推测感染触发TAO自身免疫的假说,其中TLR4发挥了关键作用。 结论:1. 提出感染在TAO发育过程中可触发自身免疫的假说,其中TLR4作为关键因子可激活免疫信号通路,诱导自身免疫细胞因子表达。2. 建议重新考虑牙周病与TAO之间的关系,因为它们是同一高危人群。TAO研究对牙周病严重程度的控制可能提供新的线索。3.对于TAO的治疗,血管内干预和自体细胞治疗均显示出良好的远期疗效,其中自体细胞治疗越来越流行,但还需要更多的临床比较。
