摘要
乳腺癌是妇女中最常见的癌症,近年来发病率不断上升。它是由非常不均匀的组织组成,其特点是微环境异常,调节肿瘤的进展,逃避癌症治疗。乳腺癌相关成纤维细胞(BCAFs)是乳腺癌微环境的主要细胞类型,可占肿瘤体积的80%以上。特别是,BCAFs通过与分泌生长因子、细胞因子和旁分泌相互作用相关的激活过程,诱导肿瘤的发生、增殖、侵袭和转移。治疗耐药性是乳腺癌患者治疗效果差甚至失败的主要原因。尽管最近在乳腺癌治疗方面取得了进展,但仍需要新的预后标记物和新的药物来定位关键的信号通路,以提高现有治疗方法的疗效,或降低毒性。在这个观点中,BCAFs代表了对乳腺癌的临床诊断、治疗和预后有用的标记物。本文就BCAFs在肿瘤中的作用作一综述,并描述与BCAFs作用相关的内分泌/化疗耐药过程。此外,它还指出了调节BCAFs诱导的治疗耐药性的分子和途径。最后,重点介绍了针对BCAFs的潜在治疗策略,并为乳腺癌治疗提供了新的工具。

摘要: Breast cancer is the most common cancer in women, which incidence has increased in recent years. It is constituted by very heterogeneous tissue characterized by an abnormal microenvironment regulating tumor progression and providing evasion from cancer therapies. Breast cancer-associated fibroblasts (BCAFs) are the main cell type of breast cancer microenvironment and can represent up to 80% of the tumor mass. In particular, BCAFs induce cancer initiation, proliferation, invasion and metastasis by undergoing an activation process associated with the secretion of growth factors, cytokines, and paracrine interactions. Therapy resistance is the main cause of poor therapeutic results or even failure in breast cancer patients. Despite recent advances in breast cancer management, there is a need for new prognostic markers and novel agents for targeting key signalling pathways to either improve the efficacy of the current therapies, or reduce toxicity. In this view, BCAFs represent markers useful to clinical diagnosis, therapy, and prognosis of breast cancer. This review focuses on the role of BCAFs in cancer, and describes the processes of endocrine/chemotherapy resistance linked to BCAFs action. Moreover, it points to molecules and pathways regulating therapy resistance induced by BCAFs. Finally, potential therapeutic strategies targeting BCAFs and offering new tools in breast cancer therapy are highlighted.
