与阻塞性睡眠呼吸暂停综合征相关的快动眼时相睡眠行为混乱中褪黑激素的影响:一系列案例

摘要

目的:快速动眼睡眠行为混乱(RBD),其主要临床症状为噩梦产生,这被认为是神经退行性疾病的先兆。阻塞性睡眠呼吸暂停综合征(OSAS)是已知能够激发呼吸暂停情况有关的觉醒的RBD样症状。这两种与睡眠有关的病症必须在相应数量的个体中重合,因为其在相似年龄组的患者个体中有一定的重叠率。直到现在,RBD症状合并OSAS的科学文献和事实上作为OSAS模仿RBD的研究很少。 方法:我们报告四例诊断多导睡眠监测仪监测的同时患有合并阻塞性睡眠呼吸暂停综合征的严重临床RBD综合征(AHI范围:10.1 - 53.2 /小时)。 结果:在所有的患者里用2毫克缓释褪黑激素治疗可以让至今未治疗的睡眠呼吸障碍的RBD症状在临床上得到相关改善。用Montplaisir法测定没有弛缓REM睡眠(RSWA)通过多导睡眠记录仪监测显示值范围从5.1到20.4%。令人惊讶的是,在褪黑素的PSG中RSWA值高,可能是因为阻塞性睡眠呼吸暂停综合征还未得到治疗。 结论:我们认为,在有RBD和OSAS病症的患者有助于不同程度的通过REM睡眠而来的不稳定RBD症状出现。因此,我们建议考虑一种治疗策略,包括对两种疾病的治疗,以获得最佳的治疗反应
