摘要
背景:越来越明显的是,SARS-CoV-2感染不仅仅局限于呼吸系统。肾脏除了是病毒的目标外,似乎对该病的结局也有重大影响。 方法:检索PubMed、Cochrane、Scopus和SciELO数据库,主要检索词为“SARS-CoV-2”、“COVID-19”、“慢性肾脏疾病”、“肾移植”、“急性肾损伤”和“肾功能不全”。 讨论:膜结合的血管紧张素转换酶2是SARS-CoV- 2的受体,这种相互作用可能导致肾素-血管紧张素系统(RAS)失衡,与COVID-19的更糟糕的临床表现相关,包括急性肺损伤、高炎症状态和血液学改变。在肾脏疾病的框架内,急性肾损伤的发展主要与病毒引起的免疫改变和直接细胞病变有关,导致较高的死亡率。对于慢性肾脏疾病,非终末期患者预后较差,而血液透析患者COVID-19病程较轻,可能是由于受细胞因子风暴影响的机会较低。此外,目前的情况不利于肾脏捐献和移植。COVID-19与肾移植受者免疫抑制之间的关系已被广泛讨论,以确定它是否会增加死亡率以及它如何与免疫抑制药物相互作用。 结论:肾脏和RAS在SARS-CoV-2感染中发挥基础性作用,需要更多的研究来全面了解COVID-19在肾脏疾病中造成的影响。
