轻度到中度溃疡性结肠炎的“靶向治疗”:支持这一策略的证据

背景:对慢性疾病,尤其是风湿病和糖尿病的管理现在纳入了一项“治疗到目标”战略,即治疗的目的是实现客观的结果。这也适用于溃疡性结肠炎(UC)。靶点被证明可以预防内器官功能障碍,特别是肠损伤及其并发症,最后是结肠直肠癌。最近,科学界试图在目前推荐的目标之外,进一步定义粘膜愈合和临床缓解。在UC中前瞻性研究这种方法的研究很少,而且在日常临床实践中也没有常规使用治疗-靶向(T2T)算法。 目的:我们的目的是回顾目前在UC中进行T2T治疗的证据,并讨论其在常规临床实践和临床试验中的应用。 方法:从2020年2月进行PubMed搜索,通过确定已发表的论文,调查UC的目标达成率。 结果:轻中度UC通过批准的药物可达到不同的目标;与长期预后相关,组织学缓解正成为一个强有力的目标。 结论:需要进一步的研究来比较T2T治疗策略和传统治疗,特别是在轻度到中度疾病谱系中。
