有证据表明,根治性前列腺切除术(RP后)失禁不仅仅是外部尿道括约肌机制不足。多达三分之一的RP后尿失禁的男性患有膀胱功能障碍,即膀胱过度活动症(OAB)。 OAB是一种复杂的症状综合征,病理生理学定义不清。它给患者带来了沉重的负担,对他们的生活质量产生了负面影响,其管理可能很困难且具有挑战性。 RP后OAB的发生率在15.2至37.8%之间。病因是多方面的,包括膀胱部分分散,逼尿肌功能不全,膀胱出口梗阻以及压力性尿失禁(SUI)并存。 RP后SUI可能导致膀胱功能失调和尿道静脉反射的激活,进而使RP后尿失禁恶化。 RP-OAB后男性的诊断检查应旨在识别潜在的病因,并对治疗方案进行个性化设置。到目前为止,还没有来自文献的可靠的数据来进行RP后OAB管理。似乎抗胆碱能药和PDE5抑制剂可有效改善OAB参数。
