Case Report

肝移植术后复发性肝细胞癌患者的免疫检查点抑制剂:一例报道及文献复习

背景:研究了免疫检查点调节剂,例如程序性死亡蛋白-1(PD-1)/程序性死亡配体-1(PD-L1)抑制剂,细胞毒性T淋巴细胞相关抗原4(CTLA-4)抑制剂。肝细胞癌(HCC)的令人鼓舞的结果。但是,这种策略在既往肝移植(LT)患者中的安全性尚未得到很好的研究。 目的:探讨LT背景下免疫检查点抑制剂在复发和转移性肝癌患者中的安全性和可行性。 方法:描述了一例始发PD-1抑制剂的LT后复发,难治性转移性HCC患者,并复习了相关文献。 结果:注射PD-1抑制剂13个周期后,肺转移完全缓解,转移性腹膜后淋巴结对药物的部分放射学反应无肝移植排斥。至少在该患者中,PD-1抑制剂被证实在控制肿瘤进展和延长患者生存中起着重要作用。 结论:这种新药可能是一种有用的方法,可以使医生保证具有先前LT的复发性转移性HCC患者获得更好的长期生存机会。然而,由于存在急性移植排斥的风险,在同种异体移植接受者中应谨慎使用,还需要进行更大的前瞻性研究,以确定最佳的免疫调节疗法,以在保留移植肝的同时达到最佳的抗肿瘤功效。
