摘要
背景:炎症性肠病(IBD)分为克罗恩病(CD)和溃疡性结肠炎(UC),以胃肠道(GI)的慢性炎症为特征。复发的症状,包括腹痛,大便频率增加,食欲下降以及贫血,导致生活质量显著下降。IBD的治疗包括化疗(如皮质类固醇、硫嘌呤)、生物制剂(如针对肿瘤坏死因子的抗体、白细胞介素12/23)和手术。然而,这些疗法的疗效并不令人满意。因此,科学家正在寻找IBD治疗的新选择,以诱导和维持缓解病症。 目的:总结不同肠道细胞在IBD病理生理中的作用及干细胞在IBD治疗中的应用。 结果:近年来的研究强调先天淋巴样细胞(ILCs)和肠上皮细胞(IECs)在IBD病理生理学中的重要作用,提示这些类型的细胞可能成为IBD治疗的新靶点。此外,最近的研究表明,干细胞移植可以减轻IBD患者的炎症,而传统疗法对IBD有耐药性。 结论:造血干细胞(hematopoietic stem cells, HSCs)和间充质干细胞(mesenchymal stem cells, MSCs)都具有修复受损组织和调节免疫系统的功能。自体造血干细胞移植可以消除自体反应细胞,用新的t细胞替代它们,从而导致长期缓解。而骨髓间充质干细胞移植对IBD的主要并发症之一肛周瘘是有效的治疗方法。
