摘要
炎症性肠病是一种免疫介导的慢性炎症性疾病。虽然病因尚不清楚,但有明显的黏膜免疫反应失衡,部分是由遗传易感性和肠道微生物失调造成的。充分抑制炎症活动并维持这种抑制是目前治疗的主要目标。然而,糖皮质激素只适用于活动性疾病的治疗,而免疫抑制剂的作用主要局限于缓解的维持。生物制剂已经广泛应用,并为IBD患者提供了治疗益处。然而,只有部分病人能从中受益。因此,迫切需要开发治疗IBD的新物质。外泌体是最近发现的纳米大小的脂质囊泡。它们由所有活细胞分泌,然后分布在各种人体体液中。据报道,不同细胞内外泌体分泌的microRNAs和功能蛋白等成分参与了IBD的发病机制。因此,外泌体有潜力成为治疗IBD的有吸引力的颗粒,作为一种无细胞治疗方法,以及诊断和监测疾病状态的生物标志物。外泌体在IBD临床应用的实用性、安全性和理想效果有待进一步研究。
